You can still apply if she has fleas but I would recommend giving her a good scrubbing with DAWN dish detergent first to rid her of any fleas.. this way you are starting clean.
If she is just itchy, applying should be fine... if her skin is obviously irritated you may want to have the vet check the area before you apply anything to it.
