When dogs don't eat dog food, it is usually because they have been given something that tastes so much better to them. However, the above diet sounds to me to be a prescription for sick doggies down the road.
For one thing, fatty foods are an invitation for pancreatitis. For another thing, dogs cannot digest dairy foods. I wouldn't go beyond the chicken, rice, very little cottage cheese, and very, very little cheese (pancreatitis and dairy). Raw baby carrots given one or two a day might be OK, but you should know that they really do not digest them very well unless they are finely shredded or chopped (whatever). That would apply to any raw vegetable. Multi-vitamins should be given daily.
However, I recommend dry dog food as well as brushing their teeth daily.
