This is just horrible for your little furbaby, however it isn't your fault. The bandage the vet used looks a lot like the bandage the hospital uses to put over a heart cath site for pressure dressing to prevent bleeding. This type of bandage is overkill for spay surgery, IMO. When both my girls had their surgery's (spay) neither one had bandages of any kind and they heal without scarring. I feel for you and your little furbaby. I pray she heals without any further problem and remember this is NOT your fault.
