I felt the exact same way when I took my Callie Lou back in October. She is a tiny girl too. I had to wait a little longer to get my Callie from my breeder because of her tiny size and waiting for her to eat well on her own. I scheduled the spaying for her and our Yorkie-Poo that we had adopted from the shelter, Maggie. I wasn't as worried about Maggie because she is so much bigger than Callie.
I had talked to my vet and told him all my concerns and he listened and reassured me....and I was fine until I had to leave that office.
I cried all the way home. They kept her overnite and she did just fine, but I was scared like you, it's completely understandable.
