I've had this problem too, but I think I've outsmarted Bilbo. I put the paste on the brush, stick it in the back of his mouth (between the teeth and gums) and let him chew on it. I rotate the brush so it gets the tops, bottoms and sides. Eventually, he will let me brush a little.
