He poo's and pee's where he sleeps - we've tried everything from pee sprays to dog litter to news paper with hormone pee spray to walking (but you heard how that goes - and we've tried with leash and harness and leash with COLLAR - he just goes INSANE) - the same with when he eats or has a toy he likes - if you attempt to pet him he tries to take your head off and acts like a tazmanian devil. =(
I wanted to add one more thing - you know he was caged all the time at the place you got him from - so that is sure to scare him and dogs don't 'show' gratitude for something....getting out of a pen shouldn't be a privledge ...his pen should be a safe place a dog wants to be in - not used as punishment.

He's going to associate that playpen with living in a tiny cage. Maybe try an ex-pen where he is still with you yet not enclosed like a mill cage.

Not to sound awful - but you seem REALLY stressed out. I'm sorry for your illness but dogs do sense stress and flipping out over his behavior will never help him - it may just make him worse. He needs patience and consistency....if that doesn't work for you - then maybe you should think of re-homing him so someone can work with him - your plate just may be too full to handle a puppy,
