He got a cleaning in april and he has a loose tooth ? did the vet not do xrays to check for bone loss in that area ? seems odd it is loose now after a cleaning as mine had theirs cleaned and any teeth with issues come out and we see a dentist - have you called the vet to find out were these a prob on xrays when getting dental?
