Not neccessarily. It's often the case though because generally, when a breeder is focusing on size and purposely trying to produce small dogs, they tend to let the overall health of the animal take a backseat. Therefor, many extremely small dogs are very ill bred and riddled with health issues.
However, tiny little ones can simply be runts and smaller, but with no health issues at all.

In either case, they smaller the dog, the more prone they are to injury, so you do have to be extremely careful.
