Make very sure that you clean up every pee/poo spot with an enzyme cleaner such as Nature's Miracle. If you don't he will always go back to those spots. I agree with Patti, if you can't hold him, have him on your lap, or aren't playing with him, he needs to be in an ex-pen or a crate. I used (actually still use) ex-pens with washable piddle pads. And remember the general rule: he will hold his pee 1 hr. per month hold he is. I even set a timer in the middle of the night to take mine out when they were puppies. Good luck.
