I would get the medium. I have the small and my 3 lb. Yorkie fits comfortably in it, but there isn't a ton of extra room. I don't think you could go wrong with the medium. We are flying to Florida tomorrow and using our Sherpa for the first time. I am a nervous wreck because I hate to fly and can't imagine what it will be like to fly with Joey. Wish us luck. But the Sherpa is a great carrier.
