The health guarantee should include the right to take the dog to a vet within the first three days that you get it and have it checked for common puppy illnesses. Then they should guarantee at least 1 year against genetic defects. Some breeders replace the pup if there is a problem (which doesn't go over too well) and some either help with vet bills or give you another pup on top of the unhealthy one (this goes for genetic defects, not worms, kennel cough, etc.).

In addition to a health guarantee, the parents should have blood panels and bile acids testing done along with hip, knee, eye and ear checks and possible thyroid panel, urinalysis, etc.
