The only thing that's ever bugged me on a questionnaire was someone who asked for emailed pictures of the inside of our home. I thought that was a tad invasive, no? I'd invite them to come over and do a home check, but I thought it was just a little odd. Had it said pictures of the yard it would've made sense, but they wanted the inside of the house.

Oh thank goodness you didn't send the pictures of the inside of your home. A good breeder would want to have many conversations with you before that point and you would want to know their name and phone number and address as well, and that would still seem weird to me.
I disagree that this tells consumers anything about the quality of a breeder by having the questionnaire. Someone could put one up, but accept every application; breeders like to "look" picky. Breeders and buyers have to "beware." Breeders can use it to weed out the worst of the worst.
