That would send up red flags for me. I would automatically think that someone had something to hide if they didn't want to fill it out.
I would definitely do it. I've been looking for Biewer breeders in the Southeast area and I've filled out a questionnaire for practically everyone before they would even begin discussing if any litters might be available in the beginning of next year (Starting research early). I think its fairly common practice. In fact, I prefer the questionaires to people just saying "well tell me about yourself". I don't know what to say and I end up writing a novel so I don't leave out anything the breeder might want to know.
Add to your questionnaire that the new owner has to send in pictures of the baby every six months/year. I saw that on one questionnaire and I thought it was a great way for you to see how the pup was being treated, and it gave the owner an opportunity to brag to someone else.
