It is most likely MVD or something altogether different.
the only way to rule out shunt with any certainly is with scintography which doesn't sound necessary right now. Since bile acids can be up with any liver functioning problem, there could be something else going on. Had you given her any drugs the week before the BAT?
Never any bladder issues, UTIs, picky eating, lethargy, excessive thirst?
In case it is MVD, the vet does need to be very careful with anesthesia and pain meds. A biopsy can be done during surgery but the best way to do it is invasive and does carry risk. Some vets would just rather skip it...
How about redoing the BAT in a month?
