you might want to try keeping her on a leash and when she gets close to the couch and ready to jump- either hold the leash tight or step on it so she can't jump up. As for after her surgery- you might want to keep her crated when you can't keep a close eye on her.
Also if she is going to jump up- try using a loud AH AH- this stops Layla dead in her tracks- but she is a very sensitiive girl- you might want to try using coins in a can and shake it when she goes to jump up and then praise her when she stays on the floor.
