I bought the medium size for my 8lb. Yorkie. The size seems perfect for her. One thing I did was teach the dog to go through the door before you put the plastic flap on, once she went thru the door on her own then I added the plastic door. I built an insert for our patio door and cut the opening for the dog door in that and the patio door closes against the insert; it worked great for our cold midwest winters.
