Have you tried chlorine bleach? I hate to even suggest it since I just posted a question on how to get away from it but it's never let me down with stains.

I've used pumice and recommended it before; it does work, until it stops working, and then nothing I've tried that will work after that. I read an advice column in the paper, and a woman had a brand new fancy toilet, and she used one of the bluing cleaners. Because the toilet wasn't flushed often, it developed a dark blue stain. She could have gotten the stain out easily with chlorine bleach, without doing any damage, but she used the pumice. She got the stain out, now it has a different stain that is much harder to come out, and can always see a slight line. The plumber said that porcelain can be scratched and once this is done, it attracts a stain to the scratched area. I agree with Carol, Comet seems to work, if no one else has ever scratched the surface.
