I think you're doing everything right.
It'll just take time. Is he tolerating the creme? Just be sure he's not trying to wipe it away with his paws. (My concern is that he doesn't hurt himself more rubbing or scratching, much like sometime we rub our eyes like crazy when we have an itch, even though we know it won't help).
