Congrats on your puppy, my Duke is turning 1 on 10/14/08 as well.
Seems like everyone answered all your questions. Just a bit on the sweater as you mentioned...when I got Duke, he was only 1.6lband the smallest sweater I found for him seems too "heavy" on him because he couldn't really walk around in it. Also, try to look for sleeve-less sweater/clothes. I find that they're so small at that size that their legs need the freedom to move where having clothes on might make it harder.
I buy pee wee pads at Walmart, it's the blue and white bag comes in 50/bag for around $13, the cheapest I've seen but I haven't look at the incontinence pads, that's a good idea too.
