Harley's tag is very small, it is a blue bone. We only had two choices for the tag, but my vet had several pink choices. On the front is the hospital name and the vaccine#, the back has our last name, Harley, and then our first name (which we paid an extra three dollars for). The vet gave me a signed document with all the other info. We live in KY. We use the puppia harness with Harley (only when we go outside) and I have the tag attached to the D ring, so he can't get to it at all. It seems like what would work in one state would work in all.
