I never did this process myself, but this paper looks more like an official receipt that the final AKC certificate will be prepared and sent to you ( or the breeder) on time.
The name Linda de Hakaway is the name the breeder gives to the dog, but , as far as I know, it's only for a reference on the dog's heritage. I too am waiting for my 3 month old yorkie's papers to arrive, and I have similar questions to make
