I don't think that they are neccessarily 'sick' if they are under 4 lbs, but the smaller they are, the more prone to injury they are. I think alot of times, you have health issues with really small Yorkies because of the people breeding them with the intention of producing really small dogs--clearly, the best interest of the dog is not the primary focus, I would think the health of the dog would just follow suit.
