Which Medi Cal are you feeding? It could be as simple as an allergy to one of the ingredients. Personally, I wouldn't add Tylan powder (or any meds) until you first try some diet elimination strategies (if your vet thinks your dog is medically stable). It can really be as simple as diet . The paw licking/chewing is a sign of allergies as well. And, fiber and probios are an option eventually too.

Have they done fecal tests then? Did she ever had giardia or anything?
