He went to the vet and was there for 6 hours and had to get a shot and had to get all of his hair cut off.

After we brought him home we replaced his play pen with an even bigger one and he loves it.
He is back to normal, only difference is that he has no hair :P

Thanks everyone so much for your support and advice- it saved my dog!
