The Ivermectin given for mites is an off label dose and in my opinion shouldn't be given unless there is a good reason to.

I have no idea about the heat cycle causing itching.

I'm not saying to not go the Ivermectin route but it would be nice to have a positive diagnosis of mites before putting her body through that. Has a skin scraping been done? Has the vet considered trying Revolution if he/she thinks it is mites? It is the regular dose but I think it is given twice within two weeks of each other. Some vets don't think it works as well as Ivermectin but if she is improving and mites seem probably, then you could go on to Ivermectin?

I don't really know but I think I would want other things looked at first like allergies...

Are you sure this isn't happening in spring/summer only?
What food is she on?
