I am so sorry.... thats just horrible!! I have no idea what u must be feeling and going through right now, but please dont look for what u might have done wrong!! You did everything right and got the help u needed asap. You are NOT to blame it is just one of them horrible things and i am so sorry for you and your daughter she muct be hurting bad right now
