I wouldn't think you would need to go back to square one. They are allowed an accident or two ... but, if this is a pattern she is beginning to repeat, then I would think you should consider going back to the crate. I would just watch her for a day and if she continues to go to the kitchen door when she needs to "go", I would say you are well on your way to great success with potty training!
