I had my 2 pound Yorkie neutered and they gave him pain meds to bring home, probably because of his size (and he had teeth extracted). He was groggy as you described and acted off for the first day. HE SLEPT A LOT. I just held him on my lap (for hours) and let him sleep while I watched tv.
