Since the rescue group had him on a special food for urinary health, I assume he has some sort of condition/problem. Is he prone to uti's or stones?

You need to find out his medical history and take him to your vet. He needs a complete bloodwork panel and possibly an ultrasound if the rescue didn't have one done.
