While it is good that your vet is being cautious, there are so many Yorkies every year that have both done at the same time. Usually they just get baby teeth pulled at the same time. Ellie had her teeth cleaned at the time of her spay and all went well. Could your vet give antibiotics before? Would that make him/her feel better about it?
Once the incision is closed they could just drape it if they are that concerned. No teeth cleaning should be going on if there is an open cavity but there shouldn't be one when they close it... If it is oozing and still "open", wouldn't a drape take care of that?
I would have a hard time agreeing to put Ellie under twice for something that most vets will do in one surgery. There is no reason to put her through anesthesia for no reason.
By the way, Ellie didn't get an infection and she didn't go on any antibiotics at all.
