I had the same experience with my puppy. Especially during those first 5 months or so until he had all of his puppy shots and could not be set down anywhere in public for fear that he may get some disease that he wasn't protected from....is was a bit of a drag but we made it. He is also pee-pad trained and wouldn't go in public places even after his shot series was over. Finally a good friend of mine went away for the weekend and had me watch her yorkie at my place....yes, we had a yorkie sleep-over. Her dog is 4yrs old and completely trained...he would zip out the door and start pottying as soon as he was out, my puppy followed eagerly and did whatever her dog was doing. The other yorkie trained my puppy that it was "cool" to pee & poo in all sorts of places outside....not just the grass which is the only place he would go before. If you have the opportunity, get a trained friend to come show your baby the ropes. It sure worked for us and we didn't even plan it....
