I can't blame her for wanting a specialist to perform the surgery. And she probably can't help what the vet charges. You say you had an agreement w/ this lady to begin with? That is what I would honor then...the agreement that you stated. If it were me in that situation, I would offer to buy the pup back, or cover the costs of the surgery up to the price that she paid for the surgery.
Hernias can be fixed and never present another problem for the dog. However, a million things could go wrong too. I lost a dog due to hernia complications, so I have to say that IMO it can be life threatening she has every right to be concerned about the dog. however, she should not expect you to do more than what your original agreement with her was as you were upfront in presenting the issue to her from the very beginning.
