First, I suggest going to a vet that specializes in toy breeds, which you are already looking to do. Next, groomers, even the good ones, sometimes miss the ear hair, which mine did because I coincidentally took mine to the vet at the same time and she noticed mine missed some hair. She also noticed they tried to get it, she could tell, they just missed some. Perhaps my dog moved on something. One of mine had a ton of earwax in her ears when I bought her. She kept scratching so I thought of mites. I got an ear cleaning solution called Oti-soothe II to use at bath time and it took weeks of cleaning to get the ear totally cleaned out, but that was the problem. So, it possibly could be buildup. I trust my vet, so whenever an issue has come up, I tell my groomer about it. I have been through 4 and this groomer seems to really try hard to do a good job. However, I probably should take on this task myself. Although I like the breeders, once the dogs were mine, checked out fine, etc., after awhile they are on to other things
