Is he losing teeth right now? If so, that could be one of the reasons why--teething often results in some very unpleasant breath.
There could be many reasons though...could be something with what he's eating, maybe his teeth need to be cleaned. Brushing the teeth routinely is a good way to prevent the problem. You can also find liquid products that you can add to the water to help fight plaque and freshen breath as well.
Just stick with what you are doing and don't expect miracles overnight.
