It's not generally a recommended practice to have a spay done during full blown heat--it increased the chances of complications (excessive bleeding) during the procedure. Of course, as you pointed out, there are instances when special circumstances would make it a better alternative, but it's not something you'd want to do if you can avoid it.
We had a female who had to be spayed during heat and she had a horrible time recovering. Not what we wanted, but her life would have been at risk if we hadn't.
