The expanded tummy and gas could be the beginning of pancreatitis. Foods high in fat, or a lot of greasy table scraps tend to trigger pancreatitis. If they drew a blood sample (ie panel), his protein results will show up.

I know first hand about "squirts" and gas. Poor Sophie had her first attack when she was 5. The advice about extra protein in their diet is excellent, plus pumpkin and fiber additive. Probotics are a life-saver and help settle the gut. If pancreatitis is the diagnosis, do lots of reading.
