We got her at 6 weeks. Since then we have done the crate training and isolated her in one room (tile floor, not carpet) and watched her continuously, etc., etc., taking her outside to prevent accidents, giving her treats, praise, etc., and she still makes not the slightest bit of association that outside = pee & poop. We also did 7 weeks with a behaviorist, spending bigbucks.
I haven't mentioned the part about her ripping wallpaper off the wall yet, but that is entirely another issue....
