Hi, welcome to YT.
Yes, dogs with floppy ears are more prone to infections. The air can't get in there as well to keep them dry.

I'm not a fan of doggie doors because I feel it is too dangerous for small dogs to be outside unattended. Too many stories of them being attacked by animals or stolen by humans.

I don't really have any great suggestions for teaching him to come to you but at this point I would only call him to you for something he enjoys. When you want him for anything he doesn't like, go get him. Does he stay when commanded?
When training you want it to be fun when they obey not what they would consider punishment (lead or crate for him). Hopefully, someone will have more helpful suggestions. Best of luck.
