I know this isnt the only factor for Avery's full grown size but, Her mom was 2.5lbs and the dad was 3 lbs. (obviously she did not plan on breeding her tiny lil girl-accident!!.. but all 3 pups survived!!) Just curious..
When my Eury was 1lb the vet said he could do it but would prefer to wait till she was a little bit bigger, less stress for her. She got it done when she was 1.8 lbs and was just fine.

They do not need to be put under. It's simply an injection similiar to getting their shot only the needle is a little bit bigger. Eury yelped a tiny bit but then she yelps every time she gets a shot. She was totally fine after a few seconds.

As far as weight gain, it sounds like a growth spurt to me. Eury is pretty steady in her growth, about 200 grams every couple weeks, but every so often it jumps to 500 grams or more.
