I know you have been advised ump-teen ways, but when all else fails--try giving him a thump on the nose. Thump as hard as you would, if you were thumping a bug off your shoulder. This was the ultimate solution for me. When I would thump Maggie, I would also say "No Bite". She no longer even thinks about it and she is still a puppy (4 months old). This advice came to me from a dog trainer.
