I'm sure you've heard this before, but change vets! Even if you are the least bit uncomfortable with the vet, there's no reason to keep going to her. I didn't like the vet that Archie first had because I felt he didn't care enough. Our current vet doesn't touch Archie unless I allow her to. It's like if you went to a doctor and they just started touching you all over...it's icky!! Plus, they are not there to train your dog--training is YOUR preference, and they should not give you advice without asking you if they can give you their two cents, or unless you ask for advice. As for housebreaking, again, it's your preference. I'm not an advocate of crate training, and have been able to train Archie without a crate. I love Warren Eckstein, pet therapist, who has a great website (www.thepetshow.com). You might want to check it out if you have any training questions.
