while working at my last animal hospital we saw a lot of puppies with kennel cough and pnemonia. We always used a cool air humidifier. we had a rubber made container with a lid and two holes cut out of it. One on either side of the box. we took the humidifier hose and attached it to one end of the box and the other hole was left open for teh exiting air. the puppy was placed in the box with the lid on and the humidifer was attached and turned on for 15 minutes. we did this twice a day for the pup.

edit:
i can't remember if it was warm or cold air humidifier. and we also ran antibiotics with sterile water thru the humidifer (tho you can't do that at home)
