My pup was small at the time of her spay and I also didn't feel comfortable with my vet's ability to operate on her. Nyx was less than 3 lbs when she was spayed. I found an animal hospital where they routinely perform surgeries on small animals. Four months later she's completely healed, can't see or feel the scar.
As for teeth, I thought the docs would have to remove an insane amount but she only had double canines, she lost all her teeth. They sometimes swallow them by accident which is why I didn't find any loose teeth.
Good luck with your pup and only do what you feel comfortable with.
