I had a pet grooming client that had this problem. His teeth were fine but it turned out it was the dog food they were feeding him. They switched to something else and it was fine after that.
Also you may want to check the gums for gingivitis. I don't know if your Vet treated that as he likely had it if he required teeth removal.
I'll have to look up the name of the stuff that you treat it with, it is just a wash that you use for 2 weeks only.
