Having been through nursing school, I understand how much stress you are under right now, secasias.

I'm not sure what to tell you. I could join in with giving you potty-training advice, but I'm not sure that's what you want/need right now. If it is, say so. Otherwise, I'd say you've got to do what's right for you and Duchess. If you haven't got the time or energy, you haven't got it. No amount of advice is going to change that.
