Emma did this a lot as a puppy, and still does sometimes We have 2 pads in our condo.

We put a folded up towel or rug under her pads. This way, when she goes off the pad, at least it's not on the carpet. You might try that.

Also, we bought one of those frames for the pee pads. This forces them to step OVER the frame and onto the pad, and they usually have better aim ONTO the pad rather than off the edge. But...I think it just happens sometimes.

Another thing to keep in mind is to change the pads often, and pick up poop right away. If the pad's too dirty, Emma probably won't go on it, because she doesn't like stepping in her own pee. If there's poop on it, she won't use it at all.
