At this point, what I would do is simply suspend the bell from the door knob of the door that you take your puppy through to go outside. That way, he will begin associating the sound of the bell ringing with the door opening.

You'll have to be on your toes. If puppy accidentally rings the bell, RUN to open it. He'll eventually learn "Hey, this works like magic. I ring this bell and Mom comes running to let me out! Cool."
