The teeth are going to have to come out so will have to undergo anaesthesia. Not spaying is more dangerous as I have seen pyometria in females as young as 2 years of age that were not spayed yet.
4lbs is not that tiny I have had 3 1/2 lbs females spayed.
First, make sure your Vet is familiar with small toy breeds of dogs.
Do a preanesthetic blood test to be sure she can handle the anesthesia. Make sure your Vet uses Isofluorine, not halathane which I don't think they even make anymore anyway so doubt you would have to worry about that but check.
You can ask your girl be on IV. Some will also attach other monitors to be sure she is doing fine through the surgery.
Make sure it is a well experienced Vet.
