I would try feminvstr's receipe. I know she is a fabulous breeder and her advice is welcomed by everyone. This formula has been posted many times and been successful.
I have also used Gerber's baby rice cereal mixed with warm water when my Japanese Chin, chronic colitis, has a bad attack. You can dilute this with more water and use it in a syringe. It helps the diarhhea. After he accepts this for a couple of days, I add some baby chicken food from a jar. Then a few days we can move on to boiled chicken and rice which I boil in homemade (fat removed) chicken broth.
I would change vet's if this is the same one who performed surgery so young on such a tiny puppy.
