Had the same problem with my last litter. Puppy was just a bit bigger than the other 2. Thank God the biggest one was the last one. All I could think of was, I can't push in, so I have to get you out. His head was out, so I broke the sack and got him breathing before lubricating mom and helping her push out the puppy just enough to get under it's shoulders and ease him the rest of the way out.
